Waiver Form
Acknowledgment of Risk:
I, the undersigned participant, understand that participating in the Rewrite Men's Retreat 2026 involves inherent risks, including but not limited to the risk of injury, illness, or death. I voluntarily assume all such risks and take full responsibility for my participation.
Release of Liability:
In consideration of being allowed to participate in the Rewrite Men's Retreat 2026, I hereby release, waive, discharge, and covenant not to sue Romans 12two Men's Ministry, its officers, agents, employees, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in this event.
Indemnification:
I agree to indemnify and hold harmless Romans 12two Men's Ministry from any claims, damages, losses, or expenses, including legal fees, arising out of my participation in the tournament, including those arising from negligent acts or omissions.
Medical Treatment:
In the event of an injury, I consent to medical treatment as deemed necessary by the organizers or medical personnel on-site.
Acknowledgment of Understanding:
I have read this waiver and indemnification agreement in its entirety. I understand its contents and voluntarily agree to its terms.
